Eligibility: Individuals who are eligible for the $600 credit under the Medicare-approved drug discount card program must use up the credit to be eligible for this program. This program covers individuals up to 200% of the Federal Poverty Limit (Single = $18,620.00 per year; Married = $24,980.00 per year). You can't be eligible for private or public insurance reimbursement. You must be unable to afford treatment.

Note: A prescription for a 3-month supply of medication must accompany application with original physician's signature. You must also sign application attesting that information provided is complete and accurate. Attach proof of income (i.e., current federal income tax return, copy of check stubs, social security statement, pension statement, bank statements, etc.) that applies to you or other household member. Zero income must be verified in writing by physician or social worker. If approved, a maximum of 3-months medication will be sent to the physician's office. Repeat requests require a new application and a prescription attached along with current income documentation.
